Select a pixel based image and select the Live Trace option from the control panel.

In CS6 you can also select the 'Tracing' workspace from the top right menu. Also CS6 Livetrace is fantastic compared to previous versions. Once you made the trace and you're happy with it just select OBJECT>EXPAND to turn it into vector.

In CS6 the first thing to check is how complicated the image, if it is very complicated take longer to solve.
To understand this instruction, begins with a few colors image, select menu [OBJECT> Image Trace> Make].
Play with the options presented in the control panel of the instruction, immediately below the menus, can playing with a <View> and care when using <PRESET> there recalculates the image and takes some time

Depends on how complex the graphic is ... mostly just play with the preset settings and you should and something you like

Also the higher the resolution the results are better. Autotracing doesn't really work on low res images. Also it very rarely works with text unless it's a hand drawn style of text where any imperfections aren't noticeable.

You may want to adjust the contrast, color, and levels in Photoshop first. Occasionally a photoshop filter will greatly ease the transformation. It may make it easier to get a cleaner smoother conversion. It may take some playing with the Illustrator preset adjustments also. If the conversion refuses to be satisfactory, I have just created paths (drawn by hand) in Photoshop and exported them to Illustrator. Hope this helps...
