Project description

We are looking for a fold out infographic that is printed as a hard copy. This will be a 3.5 inch by 3.5 inch booklet that folds out into 4 panels, totalling 8 squares each (front and back). Each square will have one of the below 8 points on it and will fold into a Parallel Gate Fold. (example attached - please do not copy, it is just an example). Please let me know if you need further clarification. Include icons or graphics to help illustrate where space allows (no stock images)
Suggested colors for fold out in RGB (can appear in any order): 
Orange: R 220, G 90, B 35
Burgundy: R 125, G 39, B 64
Grey: R 94, G 97, B 103
Dark blue: R 44, G 63, B 80

Closing the Circle of Care National Expansion Project (CCoC NEP) is a First Nations-driven initiative supported by Canada Health Infoway’s investment to improve access, efficiency and effectiveness of community-based, digital health services across Canada.
Panel Three (Second Panel inside fold out):
CCoC NEP leverages a successful Canada Health Infoway investment project with the Cowichan Tribes to scale and spread innovative digital health solutions to First Nations Health Centres across ten Canadian provinces and territories through use of a Community Electronic Medical Record (cEMR) and integrated Mustimuhw Citizen Health Portal (MCHP)
Panel Four (Third panel inside fold out – first once all the way opened): 
The Mustimuhw cEMR was designed by First Nations for First Nations, and meets the unique requirements for First Nations community-based health service delivery.
Panel Five (Fourth Panel inside fold out – second once all the way opened): 
The Mustimuhw Citizen Health Portal (MCHP) is integrated with an Infoway certified Personal Health Record (PHR), RelayHealth, allowing citizens to be more active participants in their circle of care.
Panel Six and Seven (Fifth and Sixth panels inside fold out – last two on the right once all the way opened): 
CCoC NEP began in 2017 and the enabling technology is being implemented to: 
Improve the efficiency and quality of service for the multiple types of service providers in First Nations Health Centres 
Empower citizens to actively manage their, and their family’s health and health services; and
Contribute to the infostructure and digital presence for First Nations health services
