I also use SwishMax, because it is a lot cheaper than Flash MX and can be used to do almost everything MX can do (and can actually do a few things MX cannot do...)
For coding I use Jext, a free java text editor, because it has all the features of Notepad (lol) and highlights bits of code for me to locate errors easier.

Thats not true. Flash can do EVERY thing that swish can. Swish is a collection of premade flash effects that are customized by the user.
Also, the coding with swish is very limited, you can only code buttons and stuff.
Also, with flash mx 04 there is a plugin that adds all of the swish effects to flash.
Still, it is pretty nifty and much cheaper.

Every single logo should be build in Illustrator. If you want to add your shadow effects or your filters if that's how you do your work, you should then import your illustrator drawing(s) into Photoshop to complete the project. Keeping vector files whenever possible is always a must have for any project.
Logo Design: Illustrator
Web Design: Pencil Thumbnails > Photoshop Comps > Hand coded HTML Template. Sometimes I'll cheat with Dreamweaver, depending on the project. (DW adds too much unnecessary code for my liking).
Flash Projects: Illustrator Drawing > Export to Flash MX for the animation
It shouldn't be "What software you use" but rather how do you get your ideas to the final stage.

I use Illustrator for almost everything, Photoshop when necessary to get the job done, and SciTE/Dreamweaver for coding.
