CMYK Color Bar Issue from Adobe Photoshop

Check the picture box for your tiff file. Does it have a background color of white or is it empty?
Does the second document have the color bar over or touching another image box?
Are the black boxes just 100% K or is it an overprint of all colors?
Is something going on that confuses your trapping software?
Is something in the second document set to overprint?
If you create a for press PDF, how does it look in Acrobat Pro color separation preview?
Try creating a vector color bar.
How does the built-in color bar in Quark work?
It sounds like something is going on with the content of your second document that messes up your color bar since it works in the first document.
I know that this is obvious but it might not be your color bar, it might be your document construction.
Occasionally, badly constructed vectors will drive a RIP crazy, filled objects that are not closed, points that are filled instead of stroked, that kind of thing.
Get enough of them and the page chokes the RIP.
