To create a form in Adobe Dreamweaver do the following:

1. Open an HTML page in Adobe Dreamweaver CS5 and then click the location you want the form to start.
2. From the top menu, click "Insert" on and select "Form" . A dotted red box will display on your page.
3. Go to the button Properties window and in the "Action" field, put the URL of the script that will be processing your form. If you don’t have the Properties window displayed, press Ctrl + F3
4.Click on Insert point to "Form" and then choose the type of form element you want. Every component has its specific configurations in the Properties window.
5.Type description labels before or after the form elements.
6.Go to Insert->Form ->Button. The button is a "Submit" button by default but you can change it in the Properties window. Click Submit to submit the form.
7.Save your HTML page and then upload it to your server.
